We are often asked the following questions regarding payment for services rendered by Dr. Phelts:  

DOES THE DOCTOR ACCEPT INSURANCE?

AT WHAT COST WILL THE SERVICES RENDERED BE BILLED?

HOW MUCH TIME IS SCHEDULED FOR EACH OFFICE VISIT?

WHAT FORM OF PAYMENT IS ACCEPTED?

 

DOES THE DOCTOR ACCEPT INSURANCE? 

This is how our office works with health insurance plans:

1.  The patient pays the doctor directly for services rendered

2.  The doctor gives the patient paperwork to submit to their insurance company for reimbursement.

3.  The insurance plan reimburses the patieint directly.

 

Unfortunately, reimbursement for services rendered in this office is not available for those with coverage through an HMO, EPO, Medicare, Medicaid, Champus or other like insurance programs.  

AT WHAT COST WILL THE SERVICES RENDERED BE BILLED?  

Services are billed at a rate of $60 for each 15 minutes or $240 an hour for services rendered. 

 

HOW MUCH TIME IS SCHEDULED FOR EACH OFFICE VISIT? 

Your first visit with Dr. Phelts will be scheduled for a minimum of 45 minutes and a maximum of 90 minutes. Variability in patient presentation requires flexibility in the time required for consultation, examination and treatment. You will be billed only for time spent with the doctor. Follow-up visits are scheduled according to need and are most often 30 minutes to 1 hours in length. Our minimum follow-up office visit is 30 minutes.  

 

WHAT FORM OF PAYMENT IS ACCEPTED?  
Payment can be made by cash, check, Zelle, debit or credit card (i.e. Visa, MC), It is understood that payment is due at the time service is rendered.
